We are happy to assist patients under the care of an outside physician/fertility clinic who wish to use The Fertility Center as a testing facility only through our outside monitoring program. In order to maintain quality care and timely appointments for our own patients, we are limited on the number of outside monitoring patients we are able to see each month. If you are just starting out in this process and would like to see if we have a spot available for you, please email our outside monitoring coordinators (outsidemonitoring@mrivf.com) and provide the following information:

1. Your name and date of birth
2. The name of the clinic and physician you will be working with
3. The potential start date/timeline of your first cycle requiring monitoring
4. Plan of Care form, completed by your physician/clinic
5. All necessary orders needed for your monitoring

Once you’ve been accepted into our outside monitoring program, the following information outlines the process and what you can expect. Please keep in mind, our physicians will not be involved in your plan of care. All treatment decisions/medical advice must come directly from your ordering physician’s office.

Services offered: Ultrasounds are offered upon availability at any of our three Michigan locations (Grand Rapids, Kalamazoo, and Mason).

Ultrasounds/ultrasound guided procedures: available by appointment Monday-Friday.

• Limited Follicle Ultrasounds
• 3D Ultrasounds
• Sonohysterograms
• Mock Embryo Transfers

Orders for blood work must be taken to an outside laboratory.

Requirements:

Monitoring Services Consultation: A consultation with our monitoring services coordinator is required at the start of each treatment cycle, before any monitoring through The Fertility Center. This consultation usually lasts less than 20 minutes and can be done in the office or over the phone. Our monitoring services coordinator will gather information regarding your ordering physician/clinic and treatment plan and review requirements and expectations of the program.

Plan of Care Form: After each consultation, you will need to provide a plan of care form to your ordering physician to complete and return. A new plan of care form must be completed and returned at the beginning of each treatment cycle. This form provides our clinicians with basic health information regarding your type of treatment cycle and medications.

Orders: Your ordering physician must send a completed an order to our office at the time of scheduling. This order should include the appointment date(s) and type of ultrasound with your diagnosis code. Standing orders are acceptable. Orders should be emailed to outsidemonitoring@mrivf.com or faxed attention to ‘Outside Monitoring Coordinator’ at 616-988-2010.

Emergency/After-hours care: In the event of a medical emergency. please contact your ordering physician’s office or go directly to your local emergency room. If your ordering physician would like you to be seen, they may contact our office or submit an order for an appointment during regular office hours. Please do not call our emergency/after-hours line.
